<h2>Cognitive Behavioural Therapy (CBT) and Negative Automatic Thoughts (NATs)</h2>
<p>Cognitive Behavioural Therapy, or CBT, is a an effective psychotherapy for a wide range of emotional problems. The theory underlying CBT is that our thoughts directly affect the way we feel &#8211; if we think in depressed ways then we will feel depressed. As such, a primary aim for a CBT therapist is to help a client recognise when they&#8217;re thinking in unhelpful ways. This article is a brief overview of one method of doing exactly this. I&#8217;ll use the example of someone suffering from social anxiety, a common problem amongst people I work with as a Psychiatrist and therapist in Edinburgh.</p>
<p>The first step for the client is to start &#8220;recording their thoughts&#8221; when they feel&nbsp; anxious &#8211; that is, writing down (in brief sentences) what&#8217;s going through their head. This may sound a little strange at first, but it really is an essential part of the CBT method. Ideally the client should stop and write down what they&#8217;re thinking at the moment they&#8217;re thinking it, but writing it down at the end of the day is also acceptable. They should write down everything they were thinking of &#8211; this will usually produce quite a list of thoughts and statements and beliefs. In addition, the client should note both how they felt (physically and emotionally) and the situation in which these feelings occurred.</p>
<p>In my example, the client would record that the situation was a social gathering of work colleagues after work. His heart was racing, he felt hot and sweaty, and he was a&nbsp; little light-headed (physical feelings). He described his emotional feeling as &#8220;very anxious&#8221;. Afterwards, when back at home, he wrote down the following thoughts that&nbsp; he remembered having at the time: &#8220;I don&#8217;t know anyone very well&#8221;, &#8220;I&#8217;ve got body odour&#8221;, &#8220;They&#8217;re all friends&#8221;, &#8220;This place is too busy&#8221;, &#8220;I hate these things&#8221;, &#8220;I want to go home&#8221;, &#8220;I&#8217;m going to faint and make a fool of myself&#8221;, &#8220;I&#8217;ve got to get out of here&#8221;</p>
<p>This list of thoughts that he has recorded is a list of what a CBT therapist would call &#8220;Negative Automatic Thoughts&#8221;. They&#8217;re &#8220;Negative&#8221; in that they tend to hinder the person&#8217;s motivation and ability to engage with activities, and contribute to emotional problems. They&#8217;re &#8220;Automatic&#8221; in that they seem to occur &#8220;just like that&#8221;, popping into the person&#8217;s head as if from nowhere. Indeed, unless the person specifically focuses his attention on what he&#8217;s thinking &#8211; as in the thought-recording exercise &#8211; they may pass unnoticed. All that the person would then be aware of is a sudden feeling of anxiety and a desire to leave the situation.</p>
<p>These Negative Automatic Thoughts (or NATs) cause emotional problems (in this case, social anxiety). Thinking in this way triggers the &#8220;Fight or Flight&#8221; response to perceived threat, leading to the physical symptoms of anxiety such as a racing heart and nausea. If the person didn&#8217;t have these NATs (i.e. he didn&#8217;t think thoughts such as &#8220;I&#8217;m going to faint&#8221;) then the &#8220;Fight or Flight&#8221; response would not kick-in. No physical symptoms of anxiety would result, and he would be free to enjoy the social do.</p>
<p>Having identified the client&#8217;s NATs, the next step is to find alternative, more helpful ways of thinking about the situation. This is best done by examining the NATs for their rationality or &#8220;truthfulness&#8221;. A CBT therapist (along with the client) does this by conducting a &#8220;trial&#8221; for a particular NAT. In this example I will take the NAT &#8220;I&#8217;m going to faint and make a fool of myself&#8221; &#8211; this NAT was described by the client as the most distressing thought (in CBT parlance, this would be called the &#8220;Hot Thought&#8221;).</p>
<p>In a trial, evidence is presented &#8220;for&#8221; and &#8220;against&#8221; the party concerned. It&#8217;s the same in a trial for Negative Automatic Thoughts. So what evidence is there that the statement &#8220;I&#8217;m going to faint and make a fool of myself&#8221; is true? Very little &#8211; the client felt physically unpleasant and was anxious. And the evidence that says the thought is false? Much greater &#8211; he didn&#8217;t actually faint, he&#8217;s never actually fainted in all the times when he&#8217;s felt anxious, and it&#8217;s a well known fear of people experiencing anxiety that they will faint. And besides, there seems little evidence to suggest that even if he did faint, his colleagues would be anything other than concerned about him.</p>
<p>The verdict? That his NAT &#8220;I&#8217;m going to faint and make a fool of myself&#8221; is irrational and false.</p>
<p>Next, it&#8217;s time to identify an alternative thought that does actually fit with the evidence. How about &#8220;My heart is racing and I feel uncomfortable because I&#8217;m anxious, but I won&#8217;t faint and my anxiety will pass with time&#8221;? This seems a more accurate statement of the situation, and is clearly less likely to exacerbate his anxiety symptoms. Thinking in this way will reduce his anxiety and enable him to stay out longer with his colleagues, which in turn will help to reduce his anxiety.</p>
<p>Cognitive Behavioural Therapy (CBT) says that &#8220;we feel the way we think&#8221;. An important first step in the recovery from emotional problems is to learn to identify and challenge our irrational thoughts (or &#8220;NATs&#8221;). As a Psychiatrist and therapist in Edinburgh I have found this to be an effective (and surprisingly quick) way of relieving some of the burden of difficulties such as depression and anxiety. But it is just a first step, and most clients will benefit from a more in-depth CBT approach that addresses not just their Negative Automatic Thoughts but also their Negative Core Beliefs. <p><b>does cognitive behavioural therapy actually work?</b><br />

<p>i have been seeing a therapist, for almost a year, i have been struggling with my depression a bit although i have definitley improved since i have been going, i also have low self esteem.</p>
<p>i used to do my cbt and it was working but then i gave up and I was only doing it for a few months. i haven&#8217;t done it for almost 6 months.</p>

<p>CBT has a high success rate, something like 60/70%.  It&#8217;s helpful for mental health problems like depression, anxiety/self esteem issues and OCD.  Research has shown it is as effective as anti-depressants in treating depression, although for lots of people with moderate/severe depression a combination of both a talking therapy like CBT AND medication is best to give the best chance of recovery.</p>
<p>Im on the waiting list myself.  I have more than one diagnosis but the focus of my CBT will be on my social anxiety which is fuelled by low self esteem.  With people I&#8217;ve known who&#8217;ve said CBT hasn&#8217;t been helpful, they have mainly been people who were not motivated in the first place.  Also, a lot of people find it hard to do the exercises which include exposing yourself gradually to things you find difficult.  It&#8217;s daunting for me but I think it&#8217;s better to go through some initial stress and discomfort, if it means I&#8217;m not going to have this level of anxiety forever.</p>

<p><strong>Introduction:</strong></p>
<p>Anorexia nervosa was first described by William Gull in 1868 and is characterized by deliberate and extreme weight loss. In bulimia nervosa, episodes of overeating are followed by self-induced vomiting. There is considerable overlap between these two eating disorders.</p>
<p><strong><em>1. Anorexia Nervosa:</em></strong></p>
<p>Concerns about weight, and dieting in order to lose weight are extremely common in the general population, particularly among young women. Anorexia nervosa represents an extreme form of this behavior. Fear of being fat leads to the adoption of a starvation diet. Weight falls to at least 15% below normal, so that the body mass index (BMI) is 17.5 or less. Despite this, anorexics continue to believe they are overweight, even when faced with their emaciated reflection in the mirror. This distorted body image drives them to continue to lose weight, and they may adopt other methods such as excessive exercise, self induced vomiting or abuse of laxatives, diuretics or appetite suppressants such as amphetamine. They may become preoccupied with food, hoarding it, or becoming very interested on cookery, creating elaborate meals for their family while still refusing to eat. Amenorrhea occurs in the early stages of weight loss and is an indication of a widespread endocrine disorder. Table 1 shows the signs and symptoms found in anorexia nervosa. &nbsp;</p>
<p><strong><em>2. Bulimia Nervosa:</em></strong></p>
<p>In bulimia nervosa there is also a fear of fatness, but the characteristics symptom is binge eating. &lsquo;Binges&#8217; are the consumption of huge quantities of food at a single sitting, particularly carbohydrate-rich items such as biscuits, cakes and bread. They often take place in secret, and away from meal times. Some bulimics will eat normally at other times, although calorie-controlled diets are common. A small number also have anorexia nervosa. In bulimia nervosa, binges provoke feelings of guilt and disgust and a sense of being out of control. These feelings lead to a desire to get rid of the food, usually achieved by putting fingers down the throat to induce vomiting. Many bulimics are eventually able to spontaneously vomit. As in anorexia, laxative and diuretic abuse may be further threats to health. Despite a dread of weight gain, many maintain a normal weight and may even be overweight. Menstruation is often normal.</p>
<p><strong>Epidemiology </strong></p>
<p>Bulimia is more common than anorexia nervosa. Anorexia nervosa usually starts is adolescence, and bulimia a few years later. Surveys of young women have found a prevalence of 13% for bulimia and 1 to 2% for anorexia nervosa. Both are much more common in women than men. Occupations that depend upon keeping a low body weight, such as ballet dancing and modelling, have a particularly high risk of anorexia.</p>
<p><strong>Aetiology</strong></p>
<p>The aetiology for both anorexia nervosa and bulimia nervosa is similar. There are many factors thought to be important and most cases will be due to a combination of causes.</p>
<p><strong><em>Predisposing factors</em></strong></p>
<ul>
<li>
<strong><em>Cultural factors. </em></strong><em>Anorexia nervosa and bulimia nervosa are disorders of the food-rich developed world.<br />Western society has developed a stereotyped view of physical attractiveness which equates &lsquo;thin&#8217; with &lsquo;beautiful&#8217;, and promotes negative attitudes about obesity. The media bombards us with idealised images of underweight models alongside advertisements for confectionery. Adolescents are particularly vulnerable to these cultural pressures to conform and to be attractive. </em>
</li>
<li>
<strong><em>Genetic factors.</em></strong><em> Twin studies have shown that genetic factors do play a role, probably by creating a vulnerability to weight loss so that in the presence of environmental pressures an eating disorder may develop. </em>
</li>
<li>
<strong><em>Hypothalamic dysfunction. </em></strong><em>The hypothalamic area of the brain controls feeding behaviour, temperature regulation and fluid balance. There are marked changes in the functioning of the endocrine system in anorexia (Table 1.). In the main these changes are secondary to the weight loss, but the early onset of amenorrhoea in some anorexic women suggests that some changes may be primary.&nbsp;</em>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</li>

<strong><em>Family issues. </em></strong><em>Preparing and sharing food plays an important role in family relationships. The conflicts that often arise between adolescents ant their parents can be acted out at meal times, with refusal to eat becoming an act of rebellion. There is often some abnormality in family relationships, although the problems may be a result of the eating disorder, rather than the cause of it. It is common for the mother to have some concerns about weight and dieting, and in some cases to also have an eating disorder.<strong></strong></em>

<li>
<strong><em>Psychological issues.</em></strong><em> Adolescence may be a time of conflict with parents or others. Feelings of having little control over events, lack of confidence and poor self image are common. In some cases anorexia nervosa can be a way of coping with some of these psychological pressures, by creating an illusion of being in control. Another theory is that the amenorrhoea and arrested physical development of anorexia nervosa fulfils a wish to escape the problems of adolescence and avoid adulthood. Parents who do not want their little girl to grow into a woman and leave home may collude in this illusion of prolonged childhood.<strong></strong></em>
</li>
</ul>
<p><strong>Management </strong></p>
<p>Patients with eating disorders are often very reluctant to accept that they are ill, and have the realistic fear that the main aim of treatment will be weight gain. Therefore the first challenge in managing eating disorders is engaging the patient in treatment. It may take many hours over several appointments to gain the patient&#8217;s trust, complete an assessment and built a therapeutic relationship that will allow change to begin to happen.</p>
<p>Assessment begins with a full psychiatric history and mental state examination. An informant from the family can often provide valuable information, but can only be contacted with the consent of the patient. The main psychiatric differential diagnosis to consider is depressive disorder. A detailed physical examination is important, looking for evidence of malnutrition and effects of repeated vomiting. Physical illnesses that present with weight loss must be excluded, in particular chronic debilitating diseases, malabsorption syndromes and thyrotoxicosis. Investigations may include full blood count, urea and electrolytes, creatinine, liver function tests, ECG and chest X-ray.</p>
<p>The aim of any treatment programme must be to return to a healthy weight, and stop binges and weight control measures that threaten health. It is helpful to work towards a realistic target weight that is reached through negotiation with the patient. Psychological, physical and social treatments should be considered.</p>
<p><strong><em><u>1. Psychological treatment</u></em></strong></p>
<p><em>&bull;&nbsp;&nbsp;&nbsp;&nbsp; I.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; </em><strong><em>Cognitive therapy: </em></strong><em>This has been shown to be successful in research studies. It aims to examine and change thought processes underlying the abnormal behaviour. Therapy may include keeping a diary, for example recording binges or vomiting and the thoughts and feelings that occur before, during and after this behaviour. The diary is used in therapy sessions for the patient and therapist to work together to find a strategy to change the behaviour. </em></p>
<p><em>&bull;&nbsp;&nbsp; II.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; </em><strong><em>Behavioural therapy:</em></strong><em> This depends upon the patient learning new behaviours through a system or rewards and positive feedback. Targets which are realistic and achievable are set. A plan for achieving the target is discussed, and progress is monitored. Success is rewarded with praise, and the patient gains confidence from her successes. As each target is achieved a new one is set.</em></p>
<p><em>&bull;&nbsp; III.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; </em><strong><em>Family therapy: </em></strong><em>This may be the treatment of choice if abnormal family relationships are thought to have a role in the eating disorder. There are many different models of family therapy. In most cases two therapists work together with the family. The family as a whole is seen as the source of the problems rather than the individual with the eating disorder, and it is acknowledged all members of the family will be acted out in the therapy sessions, giving the family an opportunity to understand the way the family functions and make changes. </em></p>
<p><strong><em><u>2. Physical treatment </u></em></strong></p>
<p>There is only a limited role for drug treatment in the management of eating disorders. Fluoxetine, a specific serotonin reuptake inhibitor (SSRI) which is usually used in the treatment of depression, is also used in bulimia to suppress the appetite and limit bingeing. It is not an adequate treatment for bulimia in itself and must be used alongside psychological therapist.</p>
<p><strong><em><u>3. Social treatment &nbsp;</u></em></strong></p>
<p>Some patients will require social interventions, in particular help to gain confidence and independence. Social and self-help groups, advice about housing and finances and occupational therapy may be useful.</p>
<p><strong><em><u>4. Hospital treatment</u></em></strong><em><u></u></em></p>
<p>The majority of anorexic and bulimic patients can be managed as outpatients. However, if the weight falls to a dangerously level, admission may be become necessary, ideally to the shared care of both a psychiatrist and physician. Weight gain is achieved with a diet of regular meals, supplements if necessary with high calorie drinks and snacks. The nursing staff have an important but difficult role in management. They must strike a balance between building a trusting relationship with the patient and adopting a monitoring role, supervising and meal times, ensuring there is no self-induced vomiting and recording weight gain.</p>
<p>&nbsp;</p>
<p><strong><em>Course and Prognosis &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</em></strong></p>
<p>The course of eating disorders trends to be variable and fluctuating. In general about 65% have a good outcome and maintain normal weight, 20% remain moderately underweight long term and 15% have a poor outcome, with persisting seriously low weight. Poor outcome is associated with very early or late onset of illness, a chronic course, severe weight loss, coexisting anorexia and bulimia and persisting relationship difficulties.&nbsp; Men generally have a worse prognosis.</p>

<p>To earn the love and trust of the people, listen to their problems and sympathize with them. Do not just hear them out, listen to them with your heart. Make eye contact when the person talks to you. Listen as if every word matters, and it does. Brownie points when they find out that there is a confidante in you.</p>
<p>3. Laugh out loud.</p>
<p>I do not mean that you force yourself to laugh for every joke cracked by someone, albeit you do not find it funny at all.This means finding humor in things and not being too darn serious. A person oozing with an awesome sense of humor attracts crowds and eventually, attracts success.</p>
<p>4. Don&#8217;t forget yourself.</p>
<p>In the process of fluttering around like a social butterfly, you might forget yourself, allowing everyone to push you over. Remember, love and value yourself before anyone else. If you deem yourself respectable and worthy of affection, people will flock to you and not trample on you.</p>
<p>5. Do random acts of kindness.</p>
<p>You don&#8217;t have to do a John Rockefeller and blow your savings to charity. Little acts of kindness matters the most, and this can be as simple as giving someone a surprise you-take-care card or helping an elderly cross the street. When we were kindergarten students, kindness was taught to us and greatly practiced. Now is the time to revive the good deeds and this time, let them stay for good.</p>
<p>6. Contact your old friends.</p>
<p>Sad how some friendships are destined to goodbye, but thanks to technology, you can do something about it. Relive the good old days by flipping your yearbook and look for the great people whom you want to communicate with again. Adding these old friends to your roster of support peers will surely make you feel good all over. </p>
<p>7. Develop your personality.</p>
<p>Are you grouchy, grumpy and generally morose? Whoa, you can&#8217;t go through life with those. Get rid of the bad traits and habits that perpetually hamper your growth. And really, who wants a grouchy friend anyway? </p>
<p>8. Be confident.</p>
<p>Be able to stride to the other corner of the room and introduce yourself to people with that winning smile of yours. Just remember: be confident, not arrogant.</p>
<p>9. Practice control.</p>
<p>When angry, don&#8217;t snap at anyone. Never throw a tantrum. Stay calm and collected. Be adult enough to take control of situation and transform your anger into something more productive and passive. As soon as people think your anger goes to volcanic proportions easily, they will find it hard to come to you.</p>
<p>10. Keep nurturing your relationships.</p>
<p>Your relationship with your family, friends and significant others is too precious that you must not neglect it whatever happens. Go out and have fun with them. Do things together. Happiness will never fly from your side as long as the people who matter the most are close to you.</p>
<p>In the end, using people for self-empowerment means becoming a better and more lovable person. It&#8217;s a win-win situation: the people know they can turn to you anytime and vice versa.</p>

<p>I&#8217;m am a 20 yr old woman who has always had low self-esteem and confidence issues as a child because I&#8217;ve never had family to confide in. I have now been with my loving bf for about 2years who i love with all my heart but complains that I&#8217;m not open with him. Its not that um hiding something its that I don&#8217;t know how to be open. I feel worthless, ugly, and disregarded on the inside but he reassures me that I&#8217;m worthy of everything. Why can I take this to heart and make the change that is needed? Are there any other women experiencing this too and how did you get through that episode?

<p>I have been there. I am now a 29 year old woman and things get better, I promise! I have a poor family relationship and find it very difficult to trust anyone, including my loving husband. I have had poor self-esteem all my life due to my mom always berating me about my weight while growing up. I am not fat, but thought I was for years thanks to my &#8220;loving&#8221; mother.<br />
While you may never feel that you can fully expose yourself to someone, you can get help. I started going to counseling ( I went through many counselors before finding the right one for me ) and learned a lot more about myself. My husband is frustrated, but also very patient about how I am even though he can&#8217;t fully understand where I am coming from. </p>
<p>Please just remember that you are still very young, you have a lot of life ahead of you, so learning to love yourself is necessary. No matter how you feel, understand that in your own right, you are a Goddess and that you ARE beautiful! Obviously your hunny agrees or he wouldn&#8217;t be with you still. </p>
<p>One thing that I can ad is that your early 20&#8217;s will be very difficult, but it will get better. The closer I get to 30 the more tranquil my life has become and the more secure I feel about myself and the life I have created. Don&#8217;t fall prey to society&#8217;s version of beauty, it is askew. Look in the mirror and see the positive, they are there!</p>

<h2>Borderline Personality Disorder</h2>
<p>We develop as people as we experience different things and as we mature we become flexible enough to learn from our experiences and change our behaviour to better cope with life.&nbsp; However, people who have a personality disorder find this difficult.&nbsp; Their patterns of thinking, their feelings and behaviour remain fixed and so they will present with a much more limited range of emotions, attitudes and behaviours, thus finding it difficult to cope with everyday events.</p>
<p>As you would imagine, this can lead to distress and a fear that you do not experience life as others do.</p>
<p>BPD has been described as a pattern of instability of interpersonal relationships, self-image and feelings, impulsivity that began possibly in early adulthood as indicated by five or more of the following:</p>
<ul>
<li>Frantic efforts to avoid real or imagined abandonment</li>
<li>A pattern of unstable and intense interpersonal relationships characterised by alternating between extremes of idealisation and devaluation.</li>
<li>Impulsivity in at least two areas that is potentially self-damaging. For example spending, sex, substance misuse, reckless driving, binge eating, self harming etc.</li>
<li>Identity disturbance, markedly and persistently unstable self-image or sense of self.</li>
<li>Recurrent suicidal behaviour, gestures or threats/self harming behaviour</li>
<li>Emotional instability due to reactivity of mood. For example, irritability or anxiety which lasts only for a few hours.</li>
<li>Chronic feelings of emptiness</li>
<li>Inappropriate or intense anger or difficulty controlling anger.</li>
<li>Transient, stress related paranoid ideation or dissociative symptoms.</li>
<li>Many health professionals are sceptical about the effectiveness of treatment interventions for people with a diagnosis of personality disorder, and as such are often reluctant to accept people with this diagnosis.&nbsp; However, a range of treatment interventions are available for personality disorder, including psychological treatments and drug therapy, and there is growing evidence available on the efficacy of varying treatment approaches. In general, a combination of psychological treatments reinforced by drug therapy at critical times is thought to be the most beneficial treatment available. </li>
<li>In order for treatment to be effective it has to:</li>
<li>
<em> </em>Have<em> </em>structure</li>
<li>
<em> </em>Be focused</li>
<li>
<em> </em>Be understandable to both therapist and patient</li>
<li>
<em> </em>Be long term treatment</li>
<li>The types of psychological treatments available are as follows:</li>
</ul>
<p><strong>Dynamic psychotherapy</strong></p>
<ul>
<li>Dynamic Psychotherapy is based on a developmental model of personality.&nbsp; The aim is to understand how past events can still have influences on the present.&nbsp; This therapy is usually long term and it is essential that a good relationship is formed between the therapist and the patient as this relationship is used as a way of understanding how the patient interacts with others.</li>
</ul>
<p><strong>Cognitive Analytical Therapy (CAT Therapy)</strong></p>
<ul>
<li>CAT Therapy assumes that a set of partially dissociated self states exist and account for the features of BPD.&nbsp; It aims to put these states together to examine them in treatment as well as in life experiences.</li>
</ul>
<p><strong><br /></strong></p>
<p><strong>Cognitive Therapy (CBT)</strong></p>
<ul>
<li>CBT is a structured and collaborative therapy that is goal directed and focuses more on identifying and examining underlying beliefs than an initial reduction in symptoms. <p><strong>Dialectic Behaviour Therapy (DBT)</strong></p>

<li>Therapeutic community treatments provide intense therapeutic treatments to the patient.&nbsp; However it is the environment itself that is seen as the driving force for change</li>

<p><strong>Antipsychotic Drugs</strong></p>

<li>In trials antipsychotic drugs have had variable results.&nbsp; They are thought to reduce hostility and impulsivity.</li>

<p><strong>Antidepressant Drugs</strong></p>
<ul>
<li>Tricyclic and SSRI&rsquo;s have both been recommended in the treatment of BPD.&nbsp; They are especially thought to help such symptoms of impulsivity.</li>
</ul>
<p><strong>Mood stabilisers</strong></p>
<ul>
<li>Lithium, Carbamazepine and Sodium Valporate have been useful to treat lability of mood in those with BPD. </li>

<h2>On Self Help Books: Self-help Secrets Revealed &#8211; Part 2</h2>
<p>How to Assess Your Self-Help Needs</p>
<p>Self Help is, itself, a broad category from which to choose resources for your personal benefit:&nbsp; It is wise to assess yourself first, in terms of &#8220;needs&#8221;, &ldquo;goals&rdquo; and &#8220;problems&#8221; to be solved. Because as you work through this process you will find that more than one Online Resource may fit the bill.&nbsp; Then you are in a good position to test a number of things to see what works best. That&rsquo;s why you read reviews and access &ldquo;Inside the Book&rdquo; (which Amazon features) to determine whether this is a good resource for you.</p>
<p>Your Inner Self Knows What You Need. Pay Attention to Your Gut Feelings about whatever Resource you are Testing.</p>
<p>As Martha Stewart would say: &ldquo;This is a good thing&rdquo;.</p>
<p>Life, after all, is a sequence of trial and error, followed by trial and success. You&#8217;re going to hit the jackpot more often than not. You will get what you need.&nbsp; Ideally, of course, you want to get your needs met regularly and reduce the experimenting involved in getting there.</p>
<p>Keep in mind that you are learning a very important lesson. You are learning to TRUST YOURSELF.&nbsp; And that is worth its weight in gold.</p>
<p>As you listen to yourself and hear those subtle responses to what you are examining, your heart and intuition are hard at work leading you to exactly where you need to go. Each time you do this you are fine tuning your connection with your own best resource &ndash; Your Inner Self!</p>
<p>This brings us to our first important distinction in the assessment process.</p>
<p>To keep things simple, we are going to identify two broad categories you will pursue in terms of Self-Help. These we will call:</p>
<p>1) Categories of Distress which are Problems to be Solved; and</p>
<p>2) Categories of Achievement which are our Goals to be Achieved.</p>
<p>Under Problems to be solved, we may find symptom problems such as anxiety, depression, low self-esteem, negative thinking and so on.</p>
<p>These are the types of symptoms or problems that consistently have an adverse effect on your sense of well-being.&nbsp; We can sum these up as &#8220;Negatives we want to get away from and eliminate.</p>
<p>Under Goals to be achieved, we may find personal growth, self-improvement, positive thinking, goal setting, success conditioning and so on.&nbsp; All of these &#8220;goals&#8221; will have a positive effect&nbsp; on your sense of well-being.&nbsp; We can sum these up as Positives we want to embrace.</p>
<p>So our two broad categories are:</p>
<p>1)&nbsp;&nbsp; &#8220;Distress &ndash; Problems to be Solved&#8221; which we want to move away from and:</p>
<p>2)&nbsp;&nbsp; &#8220;Achievement &ndash; Goals to be Achieved&rdquo; which we want to move toward.</p>
<p>This is our beginning point. Your problem areas or achievement goals will fall under one of these two.</p>
<p>Is it likely that you will have more than one problem area to deal with, or more than one achievement goal to pursue?&nbsp; The answer, of course, is quite likely.&nbsp; Before you begin feeling overwhelmed, consider the following:</p>
<p>For example, under Categories of Distress, you may be experiencing anxiety and low self-esteem. Which do you tackle first?&nbsp; Or, do you tackle both of them at the same time?&nbsp; The answer is: you tackle the one that troubles you the most, because the other one will be beneficially affected as well.</p>
<p>Think about this for a minute.&nbsp; If you learn to reduce your anxiety, you will likely experience an increase in self-esteem.&nbsp; Conversely, if low self-esteem is the dominant issue, then improving your self-esteem will likely reduce your anxiety.</p>
<p>In either case, the lesser problem will be positively affected.</p>
<p>That doesn&#8217;t mean it will not need focused attention of its own.&nbsp; But as the dominant problem recedes, the next one needing your attention will step into the queue. &nbsp;Remember, your ally in this is your Intuition and Feeling Centre.&nbsp;&nbsp;This is what 25 years as a therapist and 40 years as a seeker have consistently shown me.</p>
<p>Let&#8217;s move over to &#8220;Categories of Achievement&#8221; and see if this may be true here also.</p>
<p>Suppose our goals are to increase &#8220;Positive Thinking&#8221; and apply some &#8220;Success Conditioning&#8221; to our work attitude.&nbsp; We still make the same choice.&nbsp; Which of these goals do we most desire at this particular time, given our current stage of development? You can be assured that whichever goal you choose, the other will be positively affected as well.</p>
<p>After which, that secondary goal may now press for attention, or some other as yet unseen goal could materialize.&nbsp; You can&#8217;t see what&#8217;s around the corner until you actually turn the corner. And you can&rsquo;t turn the corner until you get moving!</p>
<p>For some, this may seem rather straightforward;&nbsp; for others, this may be feel overwhelming.</p>
<p>It&#8217;s time to make another important distinction.&nbsp; Just as we&#8217;ve identified two broad categories of Self-Help requirements, we must now distinguish between two broad categories of &#8220;Persons seeking Self-Help.&#8221;</p>
<p>Category 1 will be those persons who have done some work on themselves and have a sense of what &#8220;problems&#8221; or &#8220;goals&#8221; they want to deal with next.</p>
<p>Category 2 belongs to your basic &#8220;Newbie&#8221; who has little or no experience in seeking self-help and/or doesn&#8217;t trust their ability to make good choices for themselves.</p>
<p>We can readily see that these two groups are radically different, but only on the experience scale.&nbsp; Their goals and/or problems to be solved are still part of their desire to improve their lives and their sense of effectiveness.&nbsp; On these points, they are the same as category One individuals, who also want to improve their lives.</p>
<p>So the fundamental question for both groups is essentially the same:&nbsp; &#8220;Where do I start?&#8221;&nbsp; And the answer for both groups is also the same: &#8220;Start from where you are right now!&#8221;</p>
<p>Let me explain.&nbsp; If you are a newbie, then look for resources and programs that are designed for the novice and address your current problem or goal.</p>
<p>For example, if you have an issue with anxiety, phobias, or low self-esteem then pick a program, book or audio course that addresses your dominant concern.&nbsp; On the other hand, if you&#8217;ve been engaged in <a href="http://www.kennynieves.com/personal-development">Personal Development</a> for a while, then trust that your Intuition and Feeling Centre will take you where you need to go next.</p>
<p>The most pressing need or desire, for persons in either of these groups, will make its way to the front of the line.&nbsp; That&#8217;s just the way we are built!</p>

<p>Amy and Thomas Harris wrote at least two books with &#8216;OK&#8217; in the title &#8211; first came &#8216;I&#8217;m OK &#8211; You&#8217;re OK&#8217; and then &#8216;Staying OK&#8217; and there may have been more &#8211; they&#8217;re easily the best books on psychology for the lay person, with a lot of emphasis on how to feel good about yourself (for genuine reasons) and how to relate well to other people. I&#8217;d recommend that anyone start there. </p>
<p>You might then move to Margaret Frings Keyes&#8217;s &#8216;Emotions and the Enneagram,&#8217; which goes in deeper and in many ways is more profound, but I&#8217;d start with the OK books because they&#8217;re very easy to understand (without over-simplifying or talking down to the reader) and very, very practical. </p>
